Concrete structures may fail in the following ways:

Loss of cover exposing reinforcement resulting in corrosion and failure of reinforcement.
Over stressing of weak or porous concrete resulting in cracks.
Bad construction and expansion joints in concrete structures.
Reasons Of Failure:
Using deteriorated cement or poor quality aggregate.
Mixing too much water for workability.
Improper mixing, transporting and placing of concrete.
Insufficient Compaction.
Badly constructed joints.
Improper curing.
